As the title suggests, the Vallaki Hospice doors are buggy. Particularly the two main doors on the first floor seem to want to default to unlocked either upon server reset or upon anyone descending the ladder from the attic.
This doesn't happen ALL the time so I'm not certain what causes this.
As with most rentals, if you're locked in and not a keyholder, you can visit the OOC lounge and the nearest door will unlock allowing you to escape and then it will relock upon closure. However this doesn't seem to be the case for the two doors on the main floor of the Vallaki Hospice
