When: Whenever you re-level a character and change the familiar's name or pick a different familiar.
What: Familiar's remains were in the temple at the time of using the re-level crystal, after coming back they were no longer there. Trying to summon the familiar yields the "Your familiar is dead and cannot be summoned" message.
Recreate: Drop the familiar remains somewhere and re-level a character, pick a different familiar or just change its name.
