Suggestion:
When an npc reaches the badly injured status and has no detectable targets, it should roll vs fear or flee as if under the effect of fear. Feared creatures are very unpredictable, and don't stay in the same place, so this solves the problem.

EO Made it known that this was going to be fixed/changed in an upcoming update
