A nice middle ground as suggested would be some sort of sickness/effect applied to your char that requires some measure of effort to cure. In the mean time taking some sort of negative staffs/effect until you do cure it. Either by remove curse, cure disease, or something of that nature would mean there is actually some measure of concern about being bitten/scratched by them that once it set in you would be pressed to look for a way to cure it or have to suffer under the effects.

Lycanthropy is notoriously difficult to cure. Cure disease or remove curse is simply not enough to stem the flow of the curse/disease. There's a lot of requirements for a successful cure of a victim that are better left to rp and mpc applications imo. K do like strigs idea however

Ah didn't specify, I didn't mean lycanthropy. But a different illness inflicted by them as a middle ground. So you avoid the issue of every person scratched/bitten turning, but you also have to fear getting too close to them as well and risk becoming sick that can rather hamper you. Making a nice middle ground between true infection and just nothing at all happening and being about to face the endless hoard of weres with very little concern other than death if you get overwhelmed.
