I have come across this a few times on all crafted items. Lag or when people part/join seems to disrupt the -keen attribute being put on the item being crafted. Have also had other wierd effects like items not being fully made. I always make sure to check that it has the -1 keen before i sell them otherwise i toss them in the garbage.
