After you summon the blade and engage into any combat the blade disappears a few seconds after combat has ended, even if the combat only lasts a few seconds. It doesn't unsummon, the logs don't even state that the spell unsummons, it just fades into nothing.

That's how summons work now. It's not lasting very long because the timer was shortened in line with the other summoning spells.

It will be a bit longer after next update, because it's supposed to be 1 turn + rounds per level, and right now it's only rounds per level
