Summon Monster VI in Barovia

Summon Creature VI (6) in Barovia summons a Dire Wolf Pack Leader - unfortunately, the creature doesn't always appear. The summon appears, as it joins your party on the side of the screen and it responds to commands and applies Augment Summoning feat properly. I would provide a screeenshot but I can't seem to use the tab to highlight things while screenshotting and I am unfamiliar with snipping tools. Can anyone else confirm?

Norture:
I just had a dire wolf pack leader appear randomly in the room with me and another player. I have no idea who cast the spell, but I'm pretty sure they weren't in the room with us. At first we thought maybe a DM did it, but nothing happened. It was just a summon that appeared and then disappeared.

Which areas were you both in when these happened?

Iluvatar and I were just behind the ML church (Western Outskirts) and I did it once at the Lake Shore near north Vallaki where Megan's House is, (the herbalist)

Norture:
I was in the Blue Water Inn, upstairs in a room.
