Chirurgery Kit dissapears after first using but it's says what can be used ten times. It's also dissapears if trying heal disease. I think it will also dissapears after cure poison.

Kit still dissapear. Maybe becouse it was old one and need buy new kit to check. Can't check it immediately on my character.
