Where; Anywhere you can change a cleric's domain's I presume.
When; I assume whenever, but I realized the problem at 11:17:03 UTC-7.
What; When I was trying to change one of my cleric's domains to Weave, I couldn't find it in the list, even after triple checking. I asked DM Stygian Witch, and she said it must have broken recently.
Recreate; Make a cleric, use the Cleric Domain changer and check, I suppose?
Ideas; None at this moment. I do have a chat log, which I can post, and show that it was missing for me... But it is a wall of text.
